We can only accept your insurance if we can get an authorization. We cannot get an authorization UNTIL you are present in our office to provide the required information. This can lead to patients discovering that they do not have coverage when they come to our office. At that time, you can choose to pay out of pocket for an eye exam that is not covered by insurance.
You may be required to pay a co-pay and that can not be waived, by contract with the company.
Co-Pays are due at the time of service.
Contact Lens fittings are OFTEN NOT Covered In Full by your insurance. Contact Lens Examination is an additional $55. Your insurance MAY pay some of the contact lens fitting fee.
Blue Cross/Blue Shield
Medicare. We can submit a Comprehensive Eye Exam to Medicare if Medicare is your PRIMARY insurance.
Medicare patients have a $44 copay that includes the refraction.
